At Activate Allied Health, we understand that managing chronic health conditions can impact your ability to perform everyday activities. Our occupational therapists work with Medicare-eligible patients to develop practical strategies that improve independence, manage symptoms, and enhance your quality of life. If you have a valid referral from your GP, you may be eligible for up to five bulk-billed occupational therapy sessions per calendar year.
Chronic Condition Management
We help Medicare patients manage the daily impact of chronic conditions including arthritis, diabetes, heart disease, and respiratory conditions. Our occupational therapists create practical strategies to conserve energy, reduce pain, and maintain independence in daily activities.
Falls Prevention and Balance
Through comprehensive assessment and targeted interventions, we reduce your risk of falls and improve balance, strength, and confidence in movement. This is particularly important for older adults managing chronic conditions that affect mobility and stability.
Pain Management Strategies
We develop evidence-based approaches to manage chronic pain through activity modification, joint protection techniques, ergonomic advice, and adaptive strategies that allow you to participate in meaningful activities despite pain limitations.
Energy Conservation Techniques
For patients experiencing fatigue due to chronic conditions, we teach energy conservation strategies, activity pacing, and prioritisation techniques to help you accomplish daily tasks without exhausting yourself or worsening symptoms.
Home Safety and Modifications
Our occupational therapists assess your home environment and recommend practical modifications to improve safety, accessibility, and independence. This includes advice on assistive equipment and home adaptations that support chronic disease management.
Daily Living Skills
We support Medicare patients to maintain or regain independence in essential daily activities including self-care, meal preparation, household management, and community participation despite chronic health challenges.
We love helping our clients discover a happier, healthier version of themselves in a friendly environment.
