Building Strength, Not Fear: A Back Pain Success Story

When Ned first came to Activate, he’d been living with back pain for half a decade. He’d seen multiple health professionals, tried lots of approaches, but nothing seemed to shift his symptoms and his confidence had taken a hit.

So we took a different path:

            No scary language.

No “your spine is damaged” talk.

No long list of things to avoid.

Instead, we focused on building Ned’s confidence and getting him moving again; safely, gradually, and with purpose.

Ned started with simple strength work in the gym…and now he’s running, swimming, biking, and smashing triathlons.

It can be hard to visualise how exercises in the gym can translate to real world activities. This can be especially true when dealing with chronic pain. However, your Exercise Physiologist always has a plan and bit by bit you'll return to what you love doing.

Supporting Emotional Regulation Through Physical Exercise in Teens with Special Needs

Written by Kit Chan, Physiotherapist and Mother of Jonathon who is Autistic and attends Activate twice per week for Exercise Physiology sessions.

Puberty is a period of profound change, especially for teens with special needs. During this time, the brain undergoes significant remodelling and pruning, while hormone levels such as testosterone and estrogen rise. These biological changes contribute to heightened emotional sensitivity, disrupted sleep, and challenges with focus, learning, and self-regulation.

One of the most effective ways to support teens through this transition is through regular\ physical exercise. Exercise is not just sport, it’s any movement that gets the heart pumping and the body “huffing and puffing.” This type of consistent movement triggers a cascade of beneficial hormonal and neurological responses that can support emotional regulation, physical health, and overall wellbeing.

How Exercise Supports the Brain and Emotions

During physical activity, the body releases important chemicals that can help regulate mood and stress:

Endorphins – Reduce anxiety and create a sense of calm and wellbeing.

Dopamine – Boosts motivation, pleasure, and attention.

Serotonin – Supports mood stability and emotional regulation.

Cortisol – Although known as the “stress hormone,” regular exercise helps the body become less reactive to stress, improves cortisol balance, reduces anxiety, and supports better sleep.

BDNF (Brain-Derived Neurotrophic Factor) – Enhances learning, memory, and emotional control by supporting healthy brain connections.

These natural changes can help teens experience fewer emotional meltdowns, improved coping with frustration or sensory overload, and better sleep quality.

          Making Exercise Enjoyable and Accessible

Exercise can be any activity that raises the heart rate—walking, cycling, swimming, or structured gym-based exercise. For some teens, especially those with autism, the physical sensations of exertion may initially feel unfamiliar or uncomfortable. With professional support, such as guidance from an exercise physiologist, teens can gradually learn to enjoy the positive sensations and emotional benefits that come during and after exercise.

          Why Gym-Based Exercise Works Well

Gym equipment provides stable, predictable physical support, making it easier for teens who require help with motor planning and coordination. This allows them to engage safely in meaningful movement while still gaining the full physical and emotional benefits of exercise.

          Exercise, Medications, and Metabolism

Some medications commonly prescribed for teens with special needs can affect metabolism and body weight. Regular physical exercise helps:

          • regulate insulin levels

          • reduce excess glucose storage

          • improve how muscles absorb glucose

These metabolic benefits support healthy weight, stable energy levels, and improved recovery after physical activity.

The Power of a Supportive Exercise Environment

In a supportive setting like Activate, teens receive more than just physical guidance; they experience encouragement, connection, and positive social interaction. The welcoming environment created by experienced exercise physiologists helps teens:

          • build confidence

          • engage with peers

          • practise communication and teamwork

          • feel safe, supported, and motivated

These shared experiences enhance emotional wellbeing, social confidence, and overall quality of life.

Following her son’s diagnosis of Autism Spectrum Disorder, Kit volunteered and later worked as a physiotherapist at St Paul’s College, a specialist school. In this role, she supported students with diverse needs to access physical activity and exercise within school and community settings, with a focus on participation, confidence, and overall wellbeing.

Kit has a strong interest in the role of exercise for autistic adolescents. Her son has participated in regular, structured exercise with an exercise physiologist since Year 8, and Kit has observed significant improvements in his physical health, mental wellbeing, and emotional regulation. This lived experience, combined with her clinical background and engagement with current research, informs her perspective and the work she shares in this area.

“Sciatica” is one of the most commonly misused labels in back-pain conversations. Many people assume that any pain in the leg coming from the back must be sciatica, but that’s simply not true. Real sciatica is much less common than most people believe, accounting for only

These symptoms occur because the irritated nerve affects both how the leg feels and how it functions.

           Common Misconceptions About Sciatica

            Around

two-thirds of people with low-back pain also report leg pain

, and many assume that any leg symptom equals sciatica. However, most leg pain from the back is

           non-specific

A key hallmark of true sciatica:

Pain that travels below the knee

, following a predictable nerve distribution.

However, not all sciatica travels below the knee, another important indicator is that

the leg pain is typically worse than the back pain

Why Sciatica Often Flares With Coughing or Sneezing

            Coughing, sneezing, or straining increases

           intra-abdominal pressure

, which can momentarily irritate an already affected nerve root, intensifying pain.

           Where the Pain Comes From

Sciatica can originate from different lumbar spinal levels—most commonly

           L4–L5 or L5–S1

. Each level affects a different nerve root, producing different patterns of pain, numbness, or weakness down the leg and sometimes into the foot.

The Most Common Cause: Lumbar Disc Herniation

            After ruling out other conditions,

about 85% of sciatica cases involve a herniated disc

            pressing on a nerve root.

A disc herniation is the displacement of disc material beyond the normal margins of the disc space. It occurs when the disc material, typically the softer inner material breaks through the outer ring.

The leading cause of disc herniations is genetics (Patel et al (2011) PMID: 21266637 and Battie et al (2009) PMID: 19111259). Equally there is no evidence that lifting with a bent back increases the risk of disc herniations (Swain et al (2020) PMID: 31451200 and Saraceni et al (2021) PMID: 34288926). Furthermore the spontaneous regression of herniated disc tissue can occur in between 41 and 96% of cases, and can completely resolve after conservative treatment.

Clinicians often use the straight-leg-raise test as part of assessment. A positive “crossed” straight-leg-raise test (pain in the affected leg when the opposite leg is lifted) is highly specific for nerve-root compression.

MRI or CT is considered when:

Because each person experiences different symptoms, different aggravators, and different levels of nerve sensitivity, what works for one person may not work for another. This is why guided care from a trained professional is invaluable.

           A good starting point:

Begin with movements that don’t irritate the nerve but still encourage gentle mobility and load through the spine. Although exercise routines should be individualised for each person, some exercises suitable for the initial stages of sciatica may include:

            Side bends

            Trunk rotations

            Glute bridges

            Leg swings

            Walking

            These exercises apply load

away from the irritated nerve root

, reducing mechanical tension while still promoting healthy movement and mild loading through the spine. Over time, the goal is to gradually build tolerance and restore strength and flexibility without provoking symptoms.

As symptoms settle, exercises must be progressed to include more direct loading, strength work, always tailored to the individual.

           Final Thoughts

Sciatica is a specific type of nerve-related leg pain, not just any leg pain that accompanies a sore back. Although it can be intense and limiting, most cases improve significantly with conservative management, movement, and time.

Understanding the signs, causes, and treatment options helps you navigate the condition confidently and choose the right path forward.

The 3 Best Exercises for Knee Osteoarthritis

Knee osteoarthritis (OA) affects millions worldwide, leading to pain, stiffness, and reduced quality of life. But here's the good news: exercise is one of the most effective non-pharmacological treatments for managing knee OA. In fact, recent research confirms that regular movement not only eases pain but also improves strength and daily function. So, which exercises offer the best results?

Let’s break down the top 3 exercises you should be doing to manage knee osteoarthritis.

           1. Aerobic Exercise

Aerobic exercise helps improve cardiovascular health, maintain a healthy weight, and reduce systemic inflammation, all of which can help manage OA symptoms.

           Recommendations:

            150 minutes of moderate

             or

            75 minutes of vigorous

aerobic activity per week is recommended**.

Benefits include reduced joint pain and stiffness and improved endurance.

           Best options for knee OA:

             Brisk walking

             Swimming

            Cycling (stationary or outdoors)

           **Note:

This is a general recommendation. While research hasn’t determined the exact amount of aerobic exercise required specifically for osteoarthritis, any amount can offer benefits. However, to support overall health and well-being, which can positively impact OA, the recommended guidelines remain a helpful target.

           2. Resistance Training

Strengthening the muscles around the knee, especially the quadriceps and glutes can reduce joint pain.

           Recommendations:

             Resistance training programs lasting

            5 weeks or more

             produce

            moderate improvements

             in pain, strength, and function.

Interventions under 4 weeks are generally ineffective.

            Surprisingly,

            not strongly associated

with better outcomes. This means doing

            some

training consistently over time matters more than hitting perfection.

           Strength exercises for knee OA:

            Sit-to-stands or chair squats

            Step-ups

             Leg extensions

           3. Functional Exercises

Functional exercises are movements that closely mimic the activities you perform in daily life or during recreational tasks. Functional exercises can improve your ability to carry out everyday movements like walking, standing, climbing stairs, or getting out of a chair, key areas affected by knee osteoarthritis.

           What the research says:

Moderate benefits for physical function in daily activities (ADL) and sport-specific function after 3–6 months of exercise

Resistance training improves functional outcomes, especially when programs are at least 5 weeks long.

Why Exercise Is Essential in Preventing and Managing Type 2 Diabetes

Type 2 Diabetes is a chronic condition that occurs when the body becomes resistant to insulin or fails to produce enough insulin, leading to elevated blood glucose (sugar) levels. High blood sugar levels can be dangerous for your body and even cause long term damage to your organs. It is largely preventable and manageable through lifestyle modifications, with physical activity playing a central role.

The Power of Movement: What the Research Says

Strong evidence from studies shows that regular physical activity significantly lowers the risk of developing type 2 diabetes. The landmark Diabetes Prevention Program (DPP) study found that lifestyle changes involving diet and exercise reduced the incidence of type 2 diabetes by 58 percent in high-risk individuals. Lifestyle changes outperformed medication like metformin by 27 percent.

           Furthermore physical activity can:

            · improve insulin sensitivity,

            · regulate blood glucose levels,

            · support weight management, and

            · enhance cardiovascular health.

These benefits are achieved even in the absence of significant weight loss, highlighting the independent power of exercise.

           How Much and What Kind?

The Exercise and Sports Science Australia position statement outlines clear recommendations for adults:

Aerobic exercise: At least 150 minutes per week of moderate intensity (like brisk walking or cycling), or 75 minutes of vigorous intensity.

Resistance training: Two to three sessions per week, targeting all major muscle groups.

A combination of both aerobic and resistance training yields the best outcomes.

Supervised exercise, especially when guided by an Accredited Exercise Physiologist (AEP), is particularly effective. These sessions can be tailored to each individual's health status, preferences, and any coexisting conditions.

           What can I do now?

If you have Type 2 Diabetes or think you are at risk of developing it any form of movement above what you are currently doing will help. A daily 10min walk is an achievable starting point for most people. For people who are a bit more advanced trying resistance based exercises such as sitting and standing from a chair or doing push ups against the wall are a good entry point.

Sustainable change requires more than good intentions. Behaviour change strategies such as goal setting, self-monitoring, and patient education are crucial. Regular follow-ups and a supportive environment significantly improve adherence and long-term success. Referral to an AEP for structured, evidence-based support is highly recommended.

           Final Thoughts

Exercise is not just a helpful add-on, it is an essential, evidence-backed tool in both the prevention and treatment of type 2 diabetes. Whether you’re at risk or managing the condition, moving more and moving often can transform your health outcomes and quality of life. Health professionals should make exercise a standard part of diabetes care, just like medication and nutrition.

The 3 Best Exercises for Osteoporosis Backed by Science

Osteoporosis is often called a “silent disease” because bone loss happens gradually—and without symptoms—until a fracture occurs. But while that might sound discouraging, there’s good news:

you can actively build stronger bones through the right type of exercise.

Let’s explore the three best forms of exercise for bone health, based on the latest research.

           ️‍♀️ 1. High-Intensity Resistance Training

           Why it works:

Wolff’s Law tells us that bones adapt to the stress they experience. Resistance training—especially at higher intensities—places controlled mechanical load on your skeleton, prompting bones to become denser and stronger.

A study in Osteoporosis International found that postmenopausal women who did high-intensity strength or power training saw improvements in their bone mineral density (BMD), particularly in the spine and hips. Notably,

Bones love a challenge—and impact activities like jumping provide just that. A 2021 meta-analysis revealed that

           Why it works:

Here’s something fascinating: your bones get “bored.” After about 100 repetitions of a repetitive load (like jogging), they stop responding. This is called

This explains why powerlifters—who lift heavy with long rest breaks—often have excellent BMD. Their training gives bones a strong stimulus

Rethinking Pain in Exercise: A Journey Beyond Discomfort

As an accredited exercise scientist and exercise physiologist, I've journeyed through the intricate landscape of pain and exercise. My observations have led me to a provocative yet crucial discussion point: the often exaggerated response to pain in exercise settings. This blog aims to delve into this notion, inviting reflection and professional discourse.

A Personal Observation: The Startling Revelation

Let me start with a compelling example from my practice. A client, let's call them Alex, experienced chronic knee pain. They perceived this pain as a strict limitation, creating a mental blockade against certain exercises. However, upon careful assessment and gradual exposure to specific movements, Alex's pain diminished significantly. This transformation wasn't just physical – it was deeply psychological.

It begs the question: how often is our perception of pain more a barrier than the pain itself?

Decoding Pain in Exercise: More Than Just a Signal

Pain in exercise is often a complex interplay between physical sensation and psychological response. It's not merely a signal from our bodies, but also a reflection of our mental state. Studies suggest that the perception of pain can be influenced by factors like emotional state, past experiences, and even cultural background. Recognizing the subjective nature of pain can help us approach it more constructively. For clients, this means learning to differentiate between harmful pain and discomfort that signals growth. For practitioners, it involves guiding clients through this understanding and helping them build resilience.

Psychological Barriers: Fear, Mental Blocks, and Guarding

The guarding response, while protective, can sometimes exacerbate issues by creating a cycle of fear and avoidance. This psychological barrier not only hampers physical recovery but can also lead to a decreased quality of life. The fear-avoidance model shows that fear of pain can lead to avoidance behaviors, ultimately resulting in more pain and disability. To break this cycle, both clients and practitioners should focus on graded exposure to feared activities, cognitive restructuring to address unhelpful beliefs about pain, and relaxation techniques to reduce overall stress and tension.

Navigating Discomfort: When to Push, When to Pause

The decision to push through pain or to pause and rest is nuanced and should be made carefully. Understanding the difference between the pain of injury and the discomfort of muscular effort is crucial. While the former may require rest and medical attention, the latter is often a natural part of exercise adaptation and growth. Education on this distinction is key. Encouraging clients to listen to their bodies and seek professional guidance when unsure can prevent unnecessary setbacks and foster healthy progress.

From Observation to Action: Three Practical Strategies

Implementing practical, actionable strategies can empower individuals to better manage their response to pain in exercise. These strategies, including gradual exposure, mindfulness, and professional guidance, are grounded in research and practice. They provide a roadmap for individuals to navigate their pain and fears more effectively. Implementing these strategies requires commitment and consistency.

Gradual Exposure: Start with low-impact exercises and gradually increase intensity. This approach helps in understanding and differentiating pain from discomfort.

Mindfulness Techniques:: Practicing mindfulness can aid in distinguishing between fear-induced pain perception and actual physical pain.

Professional Guidance: Consulting with exercise professionals can provide personalized strategies to navigate and manage pain effectively.

Concluding Thoughts:

            Beyond Pain, Towards Understanding

This discussion is not just about pain in the physical sense. It’s about understanding the interplay between our physical sensations and psychological responses. It's about challenging our assumptions and biases about pain and exercise.

In advocating for a nuanced view of pain in exercise, my goal is not to oversimplify or disregard the complexity of pain. It's to encourage a deeper, more comprehensive understanding and approach to exercise, pain management, and overall well-being.

We often take for granted the simple acts of daily life—walking to the shops, climbing stairs, or even maintaining our balance as we move about. Yet for many Australians, these seemingly effortless activities can be significant challenges. This is where Exercise Physiologists step in, playing a crucial role in helping people regain their independence and confidence in movement.

Exercise Physiology is a field that bridges the gap between health science and practical application. We work tirelessly to understand how the human body responds to physical activity and use this knowledge to improve people's lives. Our work goes far beyond helping athletes achieve peak performance; it's about enabling individuals from all walks of life to perform the everyday tasks that many of us barely think about.

"There's nothing quite like helping clients do things they didn't have the confidence to do before. Something as simple as walking better can make a world of difference to someone's quality of life."

This sentiment is echoed by Aislinn Gesuale, another dedicated member of our team here at Activate. She emphasises the positive impact of her role, stating,

"Being able to see how much my work can help clients reach their goals and positively influence their lives is incredibly fulfilling."

Activate Exercise physiologist Quinton Le, reflecting on his experience, notes,

"Working with people is both the most rewarding and challenging part of our job. It's really cool to work as a team to help someone achieve their goals and exceed expectations. The journey getting there, overcoming barriers, setbacks, and being patient, tends to be the biggest challenge. But it makes it all the more rewarding once you reach that point."

We often work with individuals who have faced setbacks due to injury, illness, or age-related conditions. Our role involves not just physical training but also providing emotional support and motivation. We tailor our approach to each client's unique needs and circumstances, which requires a deep understanding of the human body and spirit.

Consider the case of an older adult who has become fearful of falling after a recent stumble. As an Exercise Physiologist, we would work with this individual to improve their balance, strengthen their muscles, and increase their confidence. This might involve a combination of targeted exercises, gait training, and education about fall prevention strategies. Over time, the client may progress from being housebound to confidently taking walks in their neighbourhood - a transformation that profoundly impacts their independence and overall well-being.

Similarly, for someone recovering from a stroke, the goal might be to regain the ability to perform basic self-care tasks. An Exercise Physiologist would design a programme that focuses on improving coordination, strength, and motor control. This could involve exercises that mimic everyday movements, gradually increasing in difficulty as the client progresses. The joy of seeing a client button their shirt independently for the first time in months is immeasurable, both for the individual and the Exercise Physiologist guiding their recovery.

The field of Exercise Physiology is constantly evolving, with new research and technologies emerging regularly. This requires us to be lifelong learners, continuously updating our knowledge and skills to provide the best possible care for our clients. It combines scientific rigour with practical application, making it an exciting career choice for those passionate about health and helping others.

For those considering a career in Exercise Physiology or looking to work with organisations like Activate Allied Health, it's important to understand that this profession demands more than just a knowledge of anatomy and physiology. It requires empathy, patience, and the ability to build strong, trusting relationships with clients. Celebrating small victories and maintaining a positive outlook in the face of setbacks is crucial.

Exercise Physiologists often work as part of a multidisciplinary team, collaborating with doctors, physiotherapists, occupational therapists, and other healthcare professionals. This collaborative approach ensures that clients receive comprehensive care that addresses all aspects of health and well-being.

In conclusion, Exercise Physiologists play a vital role in enabling people to reclaim their independence and enjoy the simple pleasures of everyday movement. Their work goes far beyond the gym or sports field - it's about improving quality of life, one step at a time. As Nadunie, Aislinn, and Quinton have shared, the challenges of the job are outweighed by the immense satisfaction of helping people achieve what they once thought impossible.

For those inspired by the prospect of making such a tangible difference in people's lives, a career in Exercise Physiology offers a path that is both challenging and deeply rewarding. It's a profession that allows you to combine scientific knowledge with a genuine desire to help others, empowering people to live life to the fullest, one movement at a time.

We are actively hiring driven Exercise Physiologists to join our growing team. If you'd like to apply,

As the sun rises over Melbourne, our EPs are already preparing for a day filled with client interactions, professional development, and teamwork. Olivia Chiarello, one of our experienced Exercise Physiologists, starts her day by reviewing her schedule.

"A large part of my day is then client-facing, working with a broad range of clinical populations."

The morning usually begins with client sessions. Nadunie Piyasena, another valued EP on our team, describes these interactions: "

We guide clients through their exercises, discuss strategies like pacing activities throughout the day, provide education on pain management, and go over home exercises."

            Nadunie adds,

"Sometimes, it's just about having a general chat to help clients socialise – that's an important part of their overall well-being too."

As the day progresses, our team members juggle various responsibilities. Aislinn Gesuale, an EP with a passion for ongoing learning, emphasises the importance of staying current:

"Between client sessions, I'm often planning and updating treatment plans, and keeping up to date with the latest research about my clients' presentations. This ongoing learning is crucial in providing the best care possible."

Quinton Le, an EP on our team known for his energetic approach, shares his daily flow:

"The day always starts with greeting everyone before checking the diary. Then it's a mix of working with scheduled clients, handling emails and calls, grabbing a quick bite, chatting with colleagues, and maybe running off to a home visit."

Home visits are an essential part of many EPs' routines. These visits allow us to assess and treat clients in their own environments, providing tailored solutions for their daily lives. Adele Stevens, an EP who specialises in home-based care, elaborates on the variety in her caseload:

"In one day, I work with a variety of clients and conditions funded through NDIS, Medicare, WorkCover, and private-paying clients. This diversity keeps the job interesting and challenging."

Many of our team members are proud of the mentoring aspect of the job too. Olivia shares her experience:

"I love mentoring students. It involves writing their schedules, providing feedback, and observing their skills. It's rewarding to help shape the next generation of Exercise Physiologists."

As the day winds down, our team members often engage in professional development activities. This might involve researching specific client conditions, participating in team discussions, or attending workshops. Adele notes,

"When I have spare admin time, I complete any necessary progressions or adjustments of clients' exercise programs or their assessments. If needed, I'll conduct initial assessments and reassessments, taking various subjective and objective measures as baseline levels for each client."

The collaborative nature of our work at Activate Allied Health is evident in the way our EPs interact throughout the day. Quinton describes end-of-day activities:

"Before heading home, there's often a discussion about clients with other team members. It's a chance to share insights and ensure we provide the best possible care."

This sentiment is echoed by all team members, who value the opportunity to learn from and support each other.

As the day comes to a close, our team members reflect on their achievements and prepare for tomorrow. It's a profession that demands energy, empathy, and expertise, but the rewards are immeasurable. Each day brings new challenges and opportunities to positively impact someone's life.

In our latest article, Janine Elias, our Speech Pathologist here at Activate Allied Health shares her journey into a career in Speech Pathology, along with the challenges, rewards and key insights you need to know.

For those considering a career in Speech Pathology, the path ahead is challenging and deeply rewarding. At Activate Allied Health in Melbourne, we're privileged to work with dedicated professionals like Janine Elias, who provide valuable insights into this important field. In this article, Janine shares her journey into a career in Speech Pathology, along with the challenges, rewards and key insights you need to know.

Speech Pathology is a profession that goes far beyond helping people

            "speak better."

            As Janine explains,

"It allows me to use my scientific knowledge and my creativity to support people in communicating, which is something we may take for granted and don't know how important it is until we don't have this function anymore."

This sentiment encapsulates the essence of Speech Pathology. It's a field that combines scientific rigour with creative problem-solving, all in service of one of our most fundamental human needs: communication.

The Allied Health industry in Melbourne and across Australia has significantly changed over the past decade. Janine notes, "Many more allied health professionals and job opportunities are available compared to when I first graduated 10 years ago." This growth has been partly driven by the introduction and expansion of the National Disability Insurance Scheme (NDIS), which has increased both awareness and accessibility of speech pathology services.

A day in the life of a Speech Pathologist is diverse and engaging. It involves face-to-face and telehealth sessions with clients, writing reports and support letters, reviewing work from other professionals, creating resources, and liaising with parents, clients, and other allied health professionals. For those in senior roles like Janine, there's also the responsibility of supervising junior staff, students and developing policies to support service delivery. Having worked in the field for over 10 years, Janine plays a vital role in the development and support of our staff.

According to Janine, one of the most rewarding aspects of the job is the connection with paediatric clients.

"Having fun with them and seeing their parents see them do things for the first time, such as saying words or bringing them toys, wanting to interact with them,"

are moments that make the challenges worthwhile.

However, the profession is not without its challenges. Staying up-to-date with research, collaborating with other professionals, and continually expanding one's skillset are ongoing responsibilities. As the role of a speech pathologist evolves, so must the practitioner.

For those considering this career path, certain skills and qualities are essential. Janine emphasises the importance of

"strong rapport building skills – empathy, communicating, relating to others."

Oral and written communication skills are crucial, as is active listening. This involves not just hearing what others say, but acknowledging and building upon it. In addition, technology will significantly influence the future of Speech Pathology as it continues to evolve. Janine predicts,

"Technology will play a huge role in future SP practice. Innovations in AAC (Augmentative and Alternative Communication) and how we use Technology to support our practice, whether that be using AI to assist with report writing or engage with research, will be crucial."

For aspiring Speech Pathologists, Janine offers this advice: "

Try and do as much work experience as possible, engage with other speech pathologists to determine if this is the career for you. Watch videos, podcasts, webinars."

This hands-on approach can provide valuable insights into the day-to-day realities of the profession.

While many Speech Pathologists find areas of specialisation through their professional development and client base, the field remains broad and diverse. This allows practitioners to explore different areas of interest throughout their careers.

Working at Activate Allied Health, Janine finds fulfilment in supporting some of the population's most vulnerable clients.

"We are increasing the knowledge of those who support these clients as well as raising awareness in the community about the importance of speech and language development,"

she says. This sense of purpose and community impact is a driving force for many in the profession.

In summary, a career in Speech Pathology offers the opportunity to make a profound difference in people's lives. It's a field that demands scientific knowledge, creativity, empathy, and adaptability. While it comes with its challenges, the rewards – seeing a child communicate with their parents for the first time or helping an adult regain their ability to express themselves – are immeasurable.

Speech Pathology offers a compelling path for those drawn to a career that combines intellectual stimulation with genuine human connection. As our understanding of communication challenges grows and technology continues to evolve, the field promises to remain dynamic and impactful for years to come.

, our team of dedicated Exercise Physiologists (EPs) experiences the joy and fulfilment this profession brings daily. Through their collective insights, we'll explore the many positives of being an EP while acknowledging their honest challenges.

The most universally rewarding aspect of being an EP is witnessing client success. There's an incomparable satisfaction in seeing clients achieve their goals, especially when it comes to seemingly simple acts that profoundly impact their lives. Whether it's helping someone walk more confidently or assisting a client in reaching a long-held fitness goal, these moments of progress and achievement are the heart of what makes this profession so fulfilling.

Our EPs find great joy in building strong relationships with clients and being part of their journey towards accomplishing their goals. This interpersonal aspect of the job allows for meaningful connections and the opportunity to make a real difference in people's lives. As Adele Stevens, one of our experienced EPs, notes, these relationships often lead to discovering new areas of interest and specialisation within the field.

The dynamic nature of Exercise Physiology is another significant draw. The vast and constantly evolving field offers endless opportunities for learning and growth. This continuous development keeps the work exciting and allows EPs to expand their skills and knowledge throughout their careers. For many, this aspect of the job is invigorating and provides a sense of constant progress and achievement.

Exercise Physiology also offers the chance to work with a diverse range of clients, from athletes to individuals with chronic conditions. This variety ensures that no two days are the same, providing a stimulating work environment that challenges EPs to apply their knowledge in creative ways.

Moreover, the profession allows for a great deal of autonomy and creativity. EPs often design tailored exercise programmes that suit each client's unique needs and circumstances. This creative problem-solving aspect of the job can be incredibly fulfilling, as it allows practitioners to see the direct impact of their innovative approaches.

While the rewards are numerous, it's important to acknowledge that the profession does come with its challenges. Social fatigue can be an issue, as the job requires constant interaction with different clients throughout the day. Dealing with diverse personalities and helping clients overcome deeply ingrained beliefs about their conditions can also be challenging.

However, these challenges often serve as opportunities for growth. They push EPs to develop crucial skills such as empathy, patience, and advanced communication abilities. Many find that overcoming these hurdles makes the eventual successes all the more satisfying.

At Activate Allied Health, we strive to create an environment where our EPs can thrive and fully experience the rewards of this fulfilling profession. We believe in fostering a culture of continuous learning, mutual support, and client-centred care. This approach not only helps our team navigate challenges but also maximises the immense rewards of the profession.

The journey of an EP is one of constant growth and learning. It's a profession that demands technical knowledge, emotional intelligence, adaptability, and resilience. The satisfaction of seeing a client achieve what they once thought impossible, or the knowledge that you've made a significant difference in someone's quality of life, makes any challenges encountered along the way worthwhile.

For those considering a career in this field or EPs looking for a supportive team, remember that Exercise Physiology offers a unique opportunity to blend scientific knowledge with genuine care for others. At Activate Allied Health, we're committed to supporting our team through any challenges and celebrating the many rewards together.

In summary, while Exercise Physiology comes with its own set of challenges, the rewards far outweigh them. The opportunity to positively impact lives, engage in continuous learning, and experience the joy of client success makes this a truly fulfilling career choice.

One consistent theme among our EPs is the paramount importance of interpersonal skills. Our team emphasises that empathy, patience, and the ability to build rapport are essential in this field. These soft skills are not just nice-to-haves; they're fundamental to creating successful client outcomes. As Quinton Le, one of our most dedicated EPs, explains,

While the physical aspects of exercise physiology are undoubtedly important, our team consistently highlights the role of effective communication. This includes communicating with clients and, importantly, collaborating with colleagues and other healthcare professionals. Good interpersonal skills and the ability to contribute within an interdisciplinary team are crucial for success in this field.

In a field as dynamic as Exercise Physiology, learning never stops. Our team advises that EPs need to continue thinking critically and learning, as not all clients present textbook cases. This willingness to expand one's knowledge and adapt to new challenges is key to creating successful outcomes. The commitment to ongoing professional development benefits you as an EP and ensures that clients receive the most up-to-date and effective care.

Working with diverse clients, each with unique challenges and goals requires resilience and creativity. Our EPs acknowledge that setbacks and barriers are part of the journey when helping people achieve their goals. Sharing personal challenges and demonstrating resilience can sometimes inspire and equip clients to take on their own challenges.

Creativity is particularly important when working with younger clients. Our team notes that curiosity and innovative approaches can significantly improve engagement and outcomes, especially when working with children.

Our team offers valuable advice to those considering a career in Exercise Physiology. They encourage aspiring EPs to stay curious, question everything, and not be afraid to fail or be wrong. Exercise prescription is just the foundation; building upon that foundation is key to success in this field.
Our EPs also highlight the versatility of the profession. Adele Stevens, another one of our EPs here at Activate Allied Health, emphasises this point, saying,

"There are many different places to work and clientele to work with as an EP. If you don't enjoy one area, you can always move to another that you find more fulfilling. As the industry grows, so will the need for our services and, in turn, job opportunities."

The Essence of Movement: Rethinking Exercise Engagement

The Essence of Movement: Rethinking Exercise Engagement

In the realm of exercise science, we're perpetually confronted with evolving methodologies, cutting-edge research, and the perennial quest to optimise. human performance and health. Yet, amidst this scientific context, a foundational truth often gets obscured – the paramount importance of movement, in any form, as the cornerstone of physical well-being.

As a seasoned Exercise Physiologist and a proponent of challenging industry norms, I propose a perspective that might seem controversial to some:

"the best you can hope for with clients is that they do some form of movement, however little."

This assertion is not a capitulation to mediocrity but a call to redefine our engagement with exercise and its role in health promotion.

Consider this - A client, let's call them Alex, comes to you with a history of starting and stopping various fitness regimes, each time ending in frustration and a sense of failure. Alex's story is not unique but rather emblematic of a widespread issue – the daunting nature of conventional fitness ideals. This example foregrounds a vital conversation about the barriers to regular physical activity and the need for a paradigm shift in how we define and approach movement.

Unpacking the Proposition

At its core, this stance encourages us to reconsider what we deem "successful" in the context of physical activity. Traditional metrics of success – be it lifting heavier, running faster, or achieving a sculpted physique – while valid, do not encapsulate the broad spectrum of benefits derived from simply moving more. Movement, in its most unadorned form, encompasses a range of activities beyond structured exercise, offering accessible pathways to health for a broader audience.

           Insights from Exercise Physiology

Research consistently highlights the myriad benefits of regular physical activity, including but not limited to cardiovascular health, mental well-being, and reduced risk of chronic diseases. The Exercise & Sports Science Australia (ESSA) and similar accrediting bodies underscore the importance of physical activity at any level, advocating for movement as a non-negotiable pillar of health. Studies have shown that even minimal increases in physical activity can lead to significant health benefits. This evidence supports the notion that fostering any form of movement can be a critical step in enhancing one's quality of life.

The fitness industry, at times, can be overly prescriptive, pushing narratives that valorize extreme fitness regimes and marginalize those unable to meet these standards. This bias not only alienates a significant portion of the population but also perpetuates a false dichotomy between being a dedicated athlete and leading a sedentary lifestyle. It's essential to challenge this narrative and recognize that engagement in physical activity exists on a continuum, and every point on this spectrum offers its own set of benefits.

Encouraging a Different Perspective

Adopting this broader perspective necessitates a shift in how we, as exercise professionals, engage with our clients. It involves:

             Personalising the Approach:

Tailoring recommendations to fit the individual's lifestyle, preferences, and capabilities, ensuring that the proposed activities are sustainable and enjoyable.

            Broadening the Definition of Success:

Recognizing and celebrating progress in any form, such as increased daily steps, incorporating stretch breaks into the workday, or simply choosing to take the stairs.

            Fostering Inclusivity:

Creating an environment that welcomes individuals of all fitness levels, backgrounds, and abilities, reinforcing that there's a place for everyone in the realm of physical activity.

Conclusion

As we continue to advance in our understanding of exercise science, let us not lose sight of the fundamental principle that movement, in any capacity, is beneficial. By embracing a more inclusive and flexible approach to physical activity, we can better serve our clients and the broader community, encouraging a healthier, more active society. This perspective does not undermine the value of structured exercise programs but rather complements them, recognizing that the journey towards health and well-being is both personal and varied. In fostering an environment that celebrates all forms of movement, we pave the way for a more inclusive and holistic approach to fitness and health.

Let this serve as a call to action for my fellow professionals to engage in this dialogue, challenging our preconceptions and biases, and ultimately, enriching the lives of those we serve by advocating for movement in its most universal form.

           Disclaimer

This blog post is for sparking discussion and isn't medical advice. Always talk to a healthcare provider for exercise recommendations, especially if dealing with pain or injury. This conversation is meant to open our minds and encourage a more inclusive view of fitness, acknowledging the complexities of managing pain within the context of exercise.

Rethinking Strength Gains: A Nuanced Approach to Assessing Progress

As an accredited exercise physiologist, I’m determined to peel back the layers of conventional wisdom in our field.

Consider this: Could it be that the strength clients appear to develop over time is something they had the capacity for right from the start? It might not be as far-fetched as it seems.

Imagine Alex, who embarks on a new strength program. Initially, she lifts 20 kilograms. Six weeks in, she's proudly lifting 30 kilograms. Great progress, right? But what if, with the right mental approach, technical guidance, and optimal conditions, Alex could have managed that 30 kilograms in her initial session? It is entirely possible.

This scenario challenges us to reflect: Is Alex truly getting stronger, or is she simply unlocking an inherent capability?

To see how this could be true, let us consider some underlying factors that make it possible.

1. Neural Adaptations: Tapping Into Hidden Strength

Initial surges in strength often stem from the fact that the brain is a controlling factor in our movement, strengthening connections it uses regularly and eroding ones it doesn't - much like mastering a dance sequence – a clumsy start leading to a graceful flow.

2. The Psychological Edge: The Weight of the Mind

The mental dimension in training is profound. Initially, fear and the unknown can act as additional burdens and barriers. Yet, as familiarity sets in, these psychological barriers dissipate, often resulting in notable performance boosts. The critical query here is whether this represents a bona fide increase in physical strength or the shedding of inhibitory constraints.

3. The Conviction Effect: Placebo and Performance

The potency of belief is a force to be reckoned with. When clients have faith in a particular supplement or program, their output may surge, spurred by their convictions rather than genuine physiological change. Thus, we must discern the true catalyst of progress.

4. Understanding Rest: Misreading the Rebound

Integrating rest in training is pivotal, but is the upsurge in strength that follows truly a gain, or is it merely the body bouncing back from a state of rest?

5. The Precision of Measurement: Quantifying True Progress

Variability in performance measurements can obscure true progress. Factors such as equipment, environmental conditions, and even daily physiological fluctuations prompt us to ask: How can we accurately gauge progress?

So what can you do to push the boundaries of your inherent capabilities? Consider these approaches:

Consistent Conditions:

Aim for consistency in your training environment – lift at the same time of day and use the same equipment to better measure true strength gains.

            Incremental Overload:

Progressively increasing load ensuring each increase is manageable and takes into consideration how you feel including managing your sleep and stress

To truly understand our strength, incorporating varied methods like isometric strength assessment and adjusting our training methods based on how we are feeling might offer more definitive insights. It's about enhancing our understanding of strength training and its outcomes.

The intent here is not to undervalue our profession or the efforts of our clients but to foster a more critical and layered understanding of what constitutes 'progress.' Let's aspire to be the practitioners who question, scrutinize, and affirm that the achievements we applaud are founded on authentic physiological improvement, not merely the semblance of it.

We have an incredibly important role in unlocking what is possible with our clients. The nuance might be that the path is not always as obvious and the skill of the practitioner is equally in building relationships, understanding barriers and helping clients move past their own internal beliefs and biases to achieve progress.

*Disclaimer:

This discussion aims to ignite conversation and self-reflection, inviting both professionals and clients to examine the broader narrative surrounding strength training.*

Exercise Physiology studies how the body responds and adapts to exercise and physical activity. It is a complex and fascinating field that has important implications for our health and well-being.

           The body's response to exercise

When we exercise, our bodies undergo a number of physiological changes. The cardiovascular system works harder to pump blood throughout the body, the respiratory system works harder to deliver oxygen to the muscles, and the muscular system works harder to contract and produce movement.

These changes are necessary to meet the increased demands of exercise.
